Complete and forward copy of "Change In Employee Status" form and forward this checklist to Payroll Department

Obtain keys and other Ksan property (cell phone and Apple ID info or logged out)

Witness removal of staff property

Disable key fob

Change computer password

Change telephone message

Disable email and change pasword

Disable server/WISH login

Notify management team

Notify other staff

Notify Union (within 7 days)

Post position internally on Union boards within 7 days of vacancy for a minimum of 7 days

Post externally after 7 days if not filled internally

Temporary vacancies do not need to be posted if not expected to exceed 3 months (not to exceed 12 months)

Fill out an Exit Interview (with staff if available)

Sick Leave

Employee must obtain a doctor's note

Forward any absences over five (5) days to DMI and cc HR Manager to initiate ROE

Employer required to notify Early Intervention Program after 5 days off - give employee package

Returning From Sick Leave

Employee must give two (2) weeks notice

Employee may be required to provide a medical form stating that they are fit to return to work

Payroll Department

Remove from Benefits after 20 missed work shifts

Issue ROE (this cannot be done until the last payroll has been calculated)
